Required Employer Documents

Individual Employer

1. Copy of employer’s ID card
2. Copy of employer’s house registration
3. Copy of business registration (if any*)
4. If it is a construction contract, please attach the employment contract.
5. If the employer is not the house owner, please attach a copy of the house owner’s ID card, a copy of the house owner’s house registration and a copy of the lease agreement.

Company Employer

1. Copy of company certificate, not older than 6 months*
2. Copy of the ID card of the director who has the right to sign.
3. Copy of the house registration of the director who has the right to sign.
4. Copy of company’s house registration (if any)*
5. If it is a construction contract, please attach the employment contract.
6. If the employer is not the house owner, please attach a copy of the house owner’s ID card, a copy of the house owner’s house registration and a copy of the lease agreement.

1. Worker’s name in Thai and English
2. Worker’s date of birth
3. Worker’s frontal photo
4. Copy of previous work permit, if any*
5. Copy of previous passport, if any*
